Sawaddee Krub! ; greeting in Thai style for the celebration of Thai New Year in the past month. For this week, we will continue our story about ‘Pregnancy’ which we will focus on ovulation calculator and birth control. We understand that this may be general for some people, but it stills something that a lot of people try to understand so we would like to give brief explanation about it.

We will start with the ovulation cycle and calculator. In normal cases, the calculation is base on the menstrual cycle having the approximate of 28 days per one menstrual cycle and the ovulation will usually be on the 14th or the 12th – 16th (for people with the problem of irregular menstrual cycle, the calculation must be done in periodic ). The approximate surviving period of the oocyte is around 12-24 hours before the atresia folliculi event occurs which will happen when there is no fertilization. For sperm, its approximate surviving period is around 2-3 days. With this regard, it is a suggestion for those who want to have a child that they should have sexual relations during the period which they assess that is the ovulation period. To make it easier to understand, we would like to give the example as follows: Assuming that the first day of menstruation is on 4th April, the ovulation period will be around 16th – 20th April. If the oocyte is fertilized on 20th April, the result would be detected about 6-8 days after the fertilization if tested by blood which would be around 28th April or about 14-21 days after the fertilization if tested by urine which would be around 4th – 11th May. However, we have to understand that these dates are just estimation so we cannot define the specific date in the calculation.

As we already mentioned about giving birth in the above paragraph, in this paragraph we will talk about birth control. One of the well-known methods for the gentlemen is to use condom which in addition to preventing pregnancy, it also gives the benefit of preventing sexual diseases. For the ladies, there are the use of intrauterine devices and the oral contraceptive use (the use of birth-control pill). Today we will be focusing on the use of birth-control pill. The general birth-control pill consists of 2 types: 1) the 21 pills type and 2) the 28 pills type. The question is that is there any difference between both types. The answer for that would be it is not much difference between both types at all as both focus on the adjustment of hormones. The small difference is that the 28 pills type has the additional 7 starchy pills to complement the 28 days period, which is suitable for people who tend to forget taking their pills. There also other benefits of taking birth-control pill, in addition to preventing pregnancy, in which it will help with the skin problems like acne, make the skin looks healthier or even help with the body figure to look better. The reason for that was already mentioned as these pills will not only adjust the hormones in the pill recipient into the stage that is not suitable for any pregnancy, but also adjust any hormones in that person body that may be too little or too much to the balance resulting in solving any problems caused by hormonal imbalance. This is the reason why some women take birth-control pills although there is no concern in preventing pregnancy, but only to solve any problems which occurred from hormonal imbalance. Now we are at the part which I want to emphasize it is about the other kind of birth-control pill which is called Emergency Contraception (EC). Its explanation is already stated in its name is to use only in the emergency for example when we forget to take the general birth-control pill for 2 consecutive days or the actual use in the adverse event like rape. The EC package contains 2 pills per one package. The idea is that it must be taken within 72 hours after the sexual relation, in which the first pill should be taken as soon as possible and the second pill should be taken 12 hours after you took the first pill. EC shall give the approximate prevention percentage of 75% and if you can take the pills within 24 hours after the incident the prevention percentage will increase to 85%. But like it is said in the name that to use only in the emergency, it is stated on the drug’s label that this pills should not be taken for more than 2 times per month as the side effect of taking this pills include the stimulation of cancer cells like breast cancer, increase chance of causing the extra-uterine pregnancy, and also the problem of infertility. Usually, I would tell my patient that they should not take EC for more than 2 times within their lifetime because of its severe side effect in taking the pills in long term which usually found in the recipient group of teenagers. Please be reminded as I said, to use it only in the emergency.
